Group Benefit Calculation Form The Convention pays the group benefit premiums on behalf of all our members, local churches are expected to reimburse the Convention for these premiums. The form below will help you calculate the premiums due and remittance may be made monthly, quarterly or annually. |

Application for mortgage, loan or guarantee The Convention has an extensive program to assist churches in construction and renovation of their buildings. Policies regarding amounts, security and interest rates are included as part of the application form.
Individuals and member churches can invest in our mission and ministry by lending funds to assist with mortgages and loans.
Through the kind generosity of individuals and member churches, the Convention is able to make available mission and ministry grants to assist member churches. Local Mission Grants are available for church planting and church renewal.
This form is completed by the Treasurer and sent along with the cheques to the CBOQ office. It provides the necessary information for proper allocation of funds for benefits contributions, CBOQ givings to grants and budgets, etc. |
